SIR advocacy moves Medicare coverage forward for peripheral nerve interventions
Following multisociety advocacy by SIR and the American College of Radiology (ACR), five Medicare Administrative Contractors (CGS, NGS, Noridian, Palmetto and WPS) have finalized Local Coverage Determinations (LCDs) addressing peripheral nerve injections and procedures for chronic pain, effective Oct. 25, 2026, or Nov. 1, 2026. The final policies include coverage for genicular and trigeminal RFA and stellate/lumbar sympathectomy, along with important clarifications regarding imaging guidance and excluded pain indications. However, limitations remain, including noncoverage of genicular RFA following knee arthroplasty and thoracic/intercostal nerve blocks, and restricting genicular RFA to Kellgren-Lawrence (KL) grades 3 and 4. SIR will continue advocating for broader patient access to image-guided peripheral nerve interventions.
